Dear New Patient,

We thank you for choosing Conestoga Oral Surgery for your oral surgery needs. Our goal is to make your visit here. as comfortable as possible.

CLICK HERE and complete our SECURE, HIPAA COMPLIANT online patient registration forms. By completing your paperwork at home, this allows our team to verify insurance eligibility and help complete the registration process before you arrive to your appointment. Uploading your DENTAL and MEDICAL INSURANCE CARDS as well as a Photo ID, will also expedite the process.  If you are unable to complete these online forms, please come to our office 45 minutes prior to your scheduled time to fill out your paperwork or select the printable option on our website and complete them prior to your arrival. We can also scan all dental and medical insurance cards and a photo ID upon your arrival.  As a courtesy to you we will be happy to submit insurance claims; however, you must have your complete insurance information on the forms.

Please arrive 20 minutes prior to your scheduled time to check-in and have any necessary xrays taken by our friendly staff. If you arrive later than your appointment time you may be asked to wait longer than usual or reschedule your appointment. If for any reason you are unable to keep your appointment, please call our office as soon as possible to inform us of this change.

A payment will be necessary at the time of your appointment even if insurance coverage is expected. Patients of minor age (17 years old and under) must be accompanied by a parent or legal guardian. There are NO EXCEPTIONS to this policy.

*FOR SURGERY APPOINTMENTS ONLY*

If you wish for IV sedation for your surgical procedure a consultation for this service is necessary prior to your surgical appointment. DO NOT EAT OR DRINK for EIGHT (8) HOURS BEFORE SURGERY. Daily medications prescribed by your doctor should be taken with a sip of water unless directed not to do so. A responsible adult or legal guardian must be present in order to be sedated. They must wait for you in our office during the entire visit, drive you home, and monitor you for an additional 12 hours after surgery.
